Coroner Confirms Accidental Overdose Involving Fentanyl
The death of actress Hayden Panettiere has been officially ruled an accident, caused by the toxic effects of fentanyl, Xanax, and other drugs, the Greenville County Coroner’s Office confirmed to Variety on Tuesday, September 22, 2026. This official ruling clarifies the circumstances surrounding the passing of the 36-year-old star of “Heroes” and “Nashville,” who died on August 16 in Greenville, South Carolina.
According to the coroner’s official statement, as reported by Variety, the specific substances identified were fentanyl, 4-ANPP, Alprazolam (commonly known as Xanax), Methocarbamol (Robaxin), and Quetiapine (Seroquel). The statement explicitly noted that there were “no signs of trauma” that would have contributed to her death, reinforcing the accidental nature of the passing. Emergency services were called at approximately 1:51 p.m. on August 16 regarding a cardiac arrest, and Panettiere was found unconscious at a residence where she was temporarily staying. Despite life support efforts, she was pronounced dead at 2:32 p.m.
Fentanyl’s Role and Broader Context
Among the combination of medications found in Panettiere’s system, fentanyl is considered the most potent and most likely to result in an overdose death, ABC News reported. Fentanyl is a powerful synthetic opioid, and 4-ANPP is a chemical intermediate often used in its illegal manufacture. Alprazolam is a benzodiazepine used for anxiety and panic disorders, while Methocarbamol is a prescription muscle relaxant. Quetiapine is an antipsychotic medication prescribed for conditions such as schizophrenia, bipolar disorder, and major depressive disorder, and can also aid with sleep, according to the coroner’s office via ABC News.
The Greenville Police Department had previously confirmed that their preliminary investigation found no signs of foul play or suspicious circumstances in Panettiere’s death. An acquaintance had placed the initial 911 call. Separately, the Drug Enforcement Administration (DEA) is reportedly investigating the origin of the fentanyl, specifically whether it came from South Carolina or if Panettiere may have brought it with her from California. Sources close to the investigation told ABC News that the actress had flown from Los Angeles to South Carolina the day before her death.

Hayden Panettiere began her acting career at a young age, appearing on soap operas like “One Life to Live” and “Guiding Light.” Her breakthrough came in 2000 with the film “Remember the Titans,” starring alongside Denzel Washington.
