Incannex Healthcare Inc. has made a groundbreaking announcement in the field of obstructive sleep apnea (OSA) treatment. On July 30, 2025, the company revealed positive topline results from its Phase 2 clinical trial for IHL-42X, a novel oral drug candidate. This is a significant milestone in the pharmaceutical industry, as OSA affects more than 900 million people worldwide, yet no approved oral treatments currently exist.

Breakthrough Findings from the RePOSA Trial

The RePOSA Phase 2 trial focused on testing the safety and efficacy of IHL-42X, a fixed-dose combination of dronabinol and acetazolamide. The study enrolled 121 adult participants suffering from moderate to severe OSA. Over a 28-day period, participants were divided into groups receiving either high-dose, low-dose, or placebo versions of the drug.

The results were remarkable. The trial demonstrated a reduction in the Apnea-Hypopnoea Index (AHI), the primary measure of OSA severity, by up to 83% in the high-dose group and 79% in the low-dose group compared to placebo. Additionally, 41.2% of patients in the high-dose group experienced a reduction in AHI greater than 30%, while 14.7% achieved reductions exceeding 50%. These results indicate the potential of IHL-42X to become a transformative therapy in the treatment of OSA.

Safety and Secondary Outcomes

Beyond its efficacy, IHL-42X demonstrated an outstanding safety profile during the trial. No serious adverse events were reported, and treatment-emergent side effects were primarily mild to moderate in severity. The drug also showed statistically significant improvements in secondary endpoints, including oxygen desaturation index, sleep quality, and reduced daytime fatigue.

Joel Latham, CEO and President of Incannex, expressed his enthusiasm, stating, “The combination of statistically significant improvements across multiple endpoints and an outstanding safety profile positions IHL-42X as one of the most compelling drug candidates in the global race to bring a pharmaceutical treatment for OSA to market.”

Strategic Steps Forward

Incannex is now preparing for an End-of-Phase 2 meeting with the U.S. Food and Drug Administration (FDA) to discuss the regulatory pathway for advancing IHL-42X to Phase 3 trials. The company’s ultimate goal is to achieve commercial readiness and deliver a much-needed solution for OSA patients worldwide.

In addition to its clinical progress, Incannex has been strategically expanding its financial and operational capacity. The company recently increased its at-the-market offering program by $100 million, which will support further development of its drug candidates. Incannex has also strengthened its Clinical Advisory Board, bringing on experts like Dr. Charlene E. Gamaldo from Johns Hopkins School of Medicine and Dr. Douglas B. Kirsch, Medical Director of Atrium Health Sleep Medicine, to guide the development of IHL-42X.

Market Reaction and Financial Context

The positive trial results led to a 10% surge in Incannex’s stock price, reflecting investor optimism. However, the company still faces challenges. According to TipRanks, Incannex’s financial health score is weak, driven by ongoing profitability and cash flow issues. Despite these hurdles, the company’s liquid assets exceed its short-term obligations, providing a solid foundation for future growth.

OSA remains a massive unmet medical need, with over 900 million people globally suffering from the condition. The development of an effective oral treatment like IHL-42X could revolutionize the way OSA is managed, offering hope to millions of patients who currently rely on mechanical devices such as CPAP machines.

As Incannex continues to make strides in clinical development and regulatory planning, the pharmaceutical world will be closely watching its progress. IHL-42X has the potential to be a game-changer in the treatment of obstructive sleep apnea, marking a new era for patient care.
