Annual cancer deaths in the United States attributable to alcohol more than doubled over the last three decades, according to a comprehensive study published in The Lancet Regional Health – Americas. Researchers analyzing data from the Global Burden of Disease Study found that deaths linked to alcohol consumption climbed from 11,361 in 1990 to 23,126 in 2023, highlighting a persistent increase in alcohol-related mortality across various demographics.
Dr. Chinmay Jani, the study’s lead author and a chief clinical fellow in hematology and oncology at the Sylvester Comprehensive Cancer Center of the University of Miami, noted that while overall cancer mortality in the US has declined, the proportion of deaths specifically tied to alcohol has risen by more than 58% among adults aged 20 and older.

Demographic Impact and Cancer Types
The research indicates that the impact is not uniform across the population. Men and adults aged 55 and older experienced the most significant effects. For men, liver cancer remains the strongest link to alcohol consumption, followed by esophageal and colorectal cancers. Among women aged 55 and older, breast cancer was identified as the leading alcohol-related cause of death.
Notably, for younger adults aged 20 to 54, the study found that colorectal cancer has emerged as the leading cause of alcohol-attributable cancer mortality in men and the second leading cause in women, surpassing liver cancer in both groups. Geographic variations were also observed, with Washington, D.C., reporting the highest number of alcohol-linked cancer deaths in 2023, while Utah recorded the lowest. Researchers attribute these disparities to a combination of regional alcohol consumption patterns, socioeconomic factors, and state-level policy differences.
The Biological Mechanism
The National Cancer Institute (NCI) classifies alcohol as a risk factor for several malignancies, including cancers of the head, neck, breast, liver, and digestive tract. The mechanism involves the body’s breakdown of ethanol into acetaldehyde, a probable human carcinogen. While the liver typically converts acetaldehyde into harmless acetate, the process can be incomplete, allowing the toxin to damage DNA and interfere with essential repair mechanisms.
Furthermore, alcohol consumption is associated with oxidative stress and the depletion of vital nutrients, such as folates and vitamins A, C, D, and E, which are believed to possess cancer-protective properties. Experts also point out that alcohol can alter hormone levels, such as estrogen, which is a primary factor in the development of breast cancer.
Scientific Consensus and Industry Response
The study highlights that even moderate consumption—defined as approximately one standard drink per day—can elevate cancer risk. This finding challenges earlier beliefs regarding the potential health benefits of light drinking. Dr. Andrew Freeman, director of cardiovascular prevention and wellness at National Jewish Health, emphasized that modern science is increasingly pointing to the conclusion that there is no strictly “safe” level of alcohol consumption.
The Distilled Spirits Council of the United States has contested this view. Amanda Berger, the council’s senior vice president of science and research, stated in an email that the claim of “no safe level” is not supported by the entirety of scientific evidence and misrepresents the complex relationship between alcohol and health.
As of 2026, federal guidance in the US has moved away from specific daily drink limits, instead recommending that individuals consume less alcohol to improve overall health outcomes. Researchers suggest that reducing alcohol intake to the lowest level feasible remains the most effective strategy for mitigating individual cancer risk.
